General overviewof Herpes Simplex Virus

Herpes is a very common infection that affects people of varying ages and all genders.

  • According to the World Health Organization, about 67% of all people worldwide are infected with herpes.
  • The Centers for Disease Control and Prevention (CDC) says that 47.9 % of the US population has herpes 1 (herpes simplex virus 1) and 11.9 % has herpes 2 (herpes simplex virus 2).
  • Only a small fraction of people who have a herpes infection get a herpes outbreak; most people are asymptomatic.
  • Most people with herpes have not been officially diagnosed with herpes via testing (a swab or blood antibody test).
  • Moreover, the CDC does not recommend routine herpes testing for asymptomatic people.

Two types of herpes exist: herpes 1 (HSV-1) and herpes 2 (HSV-2).

  • The general public refers to HSV-1 and HSV-2 as "oral" and "genital" herpes, respectively.

These are the most significant similarities and differences between herpes 1 and herpes 2:

  • Once you have either type, you will never get rid of it.
  • HSV-1 and HSV-2 cause similar symptoms (local tingling, burning sensation, headache, body ache, flu-like symptoms) and signs (pimple, fluid-filled painful blisters, sores, fever, lymph node enlargement)
  • With both herpes types, the number of outbreaks will decrease over time.
  • The two types are treated with the same medication. The treatment regimen depends on the anatomic location of the virus, not the virus type. (See the treatment section of this article.)
  • The two types are treated with the same antiviral medication. It can be prescription strength topical creams (e.g. Zovirax), valacyclovir (Valtrex) oral antiviral medicine. Over the counter medication and supplements consider to have questionable efficiency
  • HSV-1 (Herpes simplex virus type 1) is more common than HSV-2 (herpes simplex virus type 2). HSV-1 affects up to 80% of people, whereas only 10%-15% of the world's population is infected with HSV-2.
  • HSV-2 is slightly easier to pass than HSV-1. That is, it is slightly more contagious.
  • People usually get HSV-1 during their childhood from friends and parents through nonsexual transmission and by oral sex. By contrast, HSV-2 is mainly transmitted after 14 years of age, and for that reason it carries the stigma of being an STD (sexually transmitted infection). But both viruses can be acquired at any age.
  • HSV-2 (regardless of its anatomic location) causes more frequent outbreaks than HSV-1.

The herpes swab test is most accurate during this stage, when the concentration in the fluid of the virus is highest. To do the test, one or two blisters are punctured with a sterile needle and fluid is collected. Proper testing is important to avoid false negative results.

What to notice in this photo

Herpes usually affects one side of the skin, the left or right. Why? When herpes is reactivated, it travels from the spinal root ganglia (where it stays suppressed by immune system) through peripheral nerves to the skin surface, and it usually affects one dermatome (an area of skin supplied by particular nerves) at a time.

What other condition looks similar?

Herpes zoster, or shingles, can present with a similar skin appearance. Though it is called "herpes," it is caused by a varicella virus, not the herpes virus. The signs and symptoms are very similar.

Cold sores,fever blisters or oral herpes

Cold sores, or oral herpes, usually refers to the anatomic area of the outbreak: the lips and nose and the inside of the mouth.

Most people get it during childhood from parents or siblings. It can be transmitted via oral sex as well and for that reason it can be classified as an STD. Both herpes types can affect the area, but usually it's caused by herpes type 1.
